Athletics Registration Information

Registration for 2025-2026 athletes is now available. Students must be registered before they can participate. Register here: 25-26 SOMS Athletic Registration Form
- Please fill out this form for any sports your student plans to participate in.
- The form allows you to add all necessary information and to sign off on the mandatory player code of conduct and concussion forms.
- PHYSICALS: Before you log in, please make sure you have a digital copy/photo of a physical form or proof of a check-up signed and dated by a physician ready to upload. The physical/check up must be completed on/after April 1, 2024 to be valid for the 2025-2026 school year. If your student athlete does not have an updated physical/ check up, please make an appointment ASAP. You will not be able to submit the registration form without uploading a photo of the physical form.
- If you need support in scheduling a physical or check up or cannot make an appointment before September, please contact Quinn Craugh, Savanna Oaks Principal, at craughq@verona.k12.wi.us.
- We are working with Pivotal Health to secure affordable and accessible physical check ups for students.

(With an updated WIAA physical form already on file, this form is completed by the parents).
Schedules:
Coaches will give players a schedule and more details as the season starts. See SOMS public calendar for dates and locations of the events.
Transportation After Practice and to and from Games:
A late bus will be available Monday - Thursday at 5:15pm to take students that need transportation home after practice.
Due to bus shortages nationwide, transportation to away games may be unavailable. We may need parents/guardians' help and/or support with carpooling to away games when needed.
